For the most part, the poor etiquette begins when you call for an appointment. First of all, the sometimes ring. One minute it works or it does not. So hopefully you can get through. Second, once you get someone on the phone, they answer it like they live there and then when you state you want to make an appointment, they put you hold for about 45minues to an hour. Even if you hang up and call back they do the same think, and wonder why you are annoyed, Third, once you get to your appointment, they ask you too many personal details out in the open that is a straight violation of HIPPA regulations, they act like they are in a club rather than a place of business and YES, slow as hell!
