Dr. David Boguslavsky, MD

I am cross posting this Adriana who works in the office made the edict that I was not to be mailed the blank intake forms that need to be filled out before the initial appointment. I made it clear I didn’t have a printer or computer, just a phone. She said I had to be able to come into the office in some capacity in order to get the forms. I told her I am immunocompromised and I’m a caregiver to my mother who has cancer and is in active chemo and i don’t go into drs offices unless truly necessary. I didn’t understand why the papers couldn’t just be mailed to me. It’s a pretty big red flag if you have an employee representing an office who feels those who are at risk for covid need to make an unnecessary trip into the office, rather than just take a few pieces of paper, place them in an envelope and put a stamp on it. I said I would pay for postage. She said in a voicemail she awaited to instruct me on office policies regarding this and would cancel my appt otherwise, all rather than… mail intake forms

Dr. Boguslavsky is very pleasant, knowledgeable, compassionate and takes time to listen and answer your questions. He puts patients at ease and I feel comfortable having him as a primary care physician.

Dr. Boguslavsky is a kind, extremely knowledgeable doctor who goes out of his way to help you He takes the time and interest that I haven't seen in a doctor in many years. During my first visit he was very thorough and took the time to get to know me and my medical history. I feel very fortunate to have found him.

This joke of a Dr thinks Physical Therapy after an approved orthopedic surgery is unnecessary. I question this dr's motives, intelligence, and qualifications. Avoid on Principle.

I recommend Dr. Boguslavsky all the time to my friends that are looking for a family doctor that welcomes parents as part of their child's team. He treats you like the intelligent adult you are and is not condescending like some other doctors.

unfit insurane doc. will sell you out for a few bucks. should lose his lisense to practice medicine for the lies he writes. Cuts people off then bills for the same services. what an idiot
