Egregious Error not disclosed

This doctor injected my brain with lidocaine, instead of contrast dye, during a procedure. I had a violent "seizure-like" episode on the table and a subsequent major stoke. The doctor never told anyone of the error he made. He finally disclosed his egregious error the following day when confronted by my surgeon.
