Hyzy refused to sign a MI state required form /doctor certificate from MI Pesticide Notification Registry( MI Dept. of Agric) and refused to give a reason, This program helps asthmatics but requires a Doctors Certificate, thanks to then state rep.Debbie Stabinow. Note; Maine and NY no DC required/can be healthy to be on their registry., In MI you have to be sick. He diagnosed as asthmatic, and took my money. Wouldn't give me a (HC permit) while caring for twin newborns, 2 yr old and 4 yr. old. I was barely able to take in air-kids equipment heavy. No heart! U of Michigan has a horrible Patient Complaint System -it was the fox guarding the hen house. I filed a complaint with the Mi dept of health - totally incompetent. Dr. Fata complaints went there and read about their horrible system.
