Dr. Ahad Shiraz, MD

Dr. Shiraz and his office staff are unethical and lack empathy. I went in yesterday for a physical required for an adventure in Peru at 12,000 feet in elevation. I provided a form which we had trouble accessing during my time with him so they said they would call me to pick it up later that day. I never received a call so I contacted them right before they closed to check in and was told that the doctor would call me the next morning. I said okay and that if I didn't receive a call I would just stop by the office because it was required for my trip. Well when I stopped by, Dr. Shiraz refused to see me despite my saying I could wait until he had one minute to spare. He refused to sign my form because an EKG wasn't conducted, which the form stated that it wasn't required. I asked if there was anything they could give me to state I was in good health and the office manager printed an unofficial letter with his e-signature and two sentences, which looked like I created and printed myself.… It didn't specify my blood pressure, heart rate, temperature, nor anything actually providing indication that I was fine medically. I asked for something more detailed and they then printed something with ALL my information, including private information that was not required in the form from years ago, which is a complete HIPAA violation and I'll be reporting them for that too. I'm honestly baffled by their unethical behavior and overall lack of compassion.
