Dr. Cassandra Susan Reyes

Cassandra “Cassie” Reyes is a family medicine nurse practitioner at Intermountain Health’s Park City Clinic – Round Valley in Heber City, Utah. She supports patients in Heber City, Park City, Kamas, Francis, and surrounding communities, with clinical interests in community health, primary prevention, cardiology, and diabetes management, with a focus on promoting long-term wellness through education, early intervention, and evidence-based care. She particularly enjoys caring for patients with chronic conditions such as hypertension, diabetes, cardiovascular disease, and chronic respiratory illnesses. These cases allow her to build long-term relationships and make a meaningful impact through education, preventive care, and ongoing support.

Cassie believes in delivering compassionate, patient-centered care that emphasizes respect, education, and empowerment. She strives to build trusting relationships, promote health equity, and support informed decision-making …through evidence-based practice. Her goal is to treat each patient as a whole person—physically, emotionally, and culturally—while advocating for their well-being at every stage of life. Her goal is to partner with patients to not only manage illness, but to promote overall wellness.

Outside the clinic, the Hawaii native enjoys backpacking, snowboarding, and running.
