Charity Ukwuoma

Charity Ukwuoma, APRN, FNP-C, PMHNP-C, is a board-certified Family Nurse Practitioner and Psychiatric Mental Health Nurse Practitioner credentialed by the American Academy of Nurse Practitioners. She obtained her initial nursing licensure from the Arizona State Board of Nursing in 2008, earned a Bachelor of Science in Nursing (BSN) from Grand Canyon University in 2011, and completed a Master of Science in Nursing (MSN) in 2015. Before becoming a Nurse Practitioner, she gained seven years of clinical experience as a Registered Nurse in a variety of healthcare settings, including Telemetry/Progressive Care, Medical-Surgical, and Post-Partum units.

With more than eight years of experience as a Family Nurse Practitioner, Mrs. Ukwuoma has provided comprehensive, patient-centered primary care to individuals across the lifespan. In addition to her primary care expertise, she holds advanced training and certification in psychiatric and mental health care, enabling her to assess, diagnose, …and treat a broad range of mental health conditions through an integrated medical and behavioral health approach. Her professional interests include Medical Weight Loss, Men’s Health, and Women’s Health, allowing her to deliver holistic care that supports both physical and emotional well-being.

Mrs. Ukwuoma is recognized for her compassionate approach to patient care and her commitment to treating every individual with respect and dignity. She is dedicated to helping patients achieve optimal health, emotional balance, and long-term wellness through evidence-based practices and personalized treatment plans. She is also an active member of the Golden Key International Honour Society, the American Association of Nurse Practitioners, and the National Association of Nigerian Nurse Practitioners (USA), reflecting her ongoing commitment to professional excellence and service.
