Inaccurate Med reconciliation, physical assessment charting falsification and errors, labs and drug interactions issues (lack of knowledge), med errors, does not return pharmacy’s calls over Med errors. Too lazy to look stuff up or do his job, would not recommend as a RN.

Does not update medication reconciliation so medication refills are incorrect. Does not look up information he does not know like lab/drug interactions so results are not accurate. Cuts and pastes physical assessments between visits and does not edit with current findings. Does not assess things charted as assessed. As a RN, would not recommend. May be friendly, but cannot just make up assessment findings and fraudulent chart.

They take forever to call you back and that’s not good I will not be seeing dr baker or this office again I only gave one star
