Second surgery with Dr Formanek. I wouldn’t be back if I didn’t trust his skills. Efficient, friendly, intelligent and he enjoys his craft. He discribed the procedure and answered all my questions. I am healing now and happy I once again I chose Dr Formanek. Office staff are friendly and attentive, as are doctor’s assistants. This gentleman is a pleasure to meet and I trust his knowledge.

He's the only one in area that actually casts feet for orthotics. He's efficient in his capacity. Suggest trying prior to any back surgeries

I visited this doctor several times, still no resolve. He wants me to pay for something that my insurance and I have both paid for already. Money hungry weirdo.
