Dr. Dorothy Delisfort-Crisostomo, DNP, PMHNP-BC, FNP-C, is a dual board-certified Psychiatric-Mental Health Nurse Practitioner and Family Nurse Practitioner with more than 28 years of nursing and healthcare experience and approximately a decade of advanced-practice experience.

As the founder and Chief Executive Officer of U-First Healthcare, LLC, Dr. Delisfort-Crisostomo provides integrated psychiatric, behavioral health, and primary care services with an emphasis on comprehensive, individualized, evidence-based treatment.

Her clinical approach recognizes the important relationship between mental and physical health. She evaluates each patient comprehensively, considering psychiatric symptoms, medical conditions, medications, sleep, stress, family and social factors, lifestyle, and individual treatment goals when developing a plan of care.

Her psychiatric practice includes comprehensive psychiatric evaluations, diagnostic assessment, medication management, …psychopharmacology, treatment monitoring, brief psychotherapeutic interventions when clinically appropriate, behavioral health care, and ongoing management of common psychiatric and mental health conditions.

As a Family Nurse Practitioner, she also has expertise in primary care, preventive health, physical assessment, health screening, chronic disease management, patient education, and coordination of care.

Dr. Delisfort-Crisostomo emphasizes collaborative decision-making and patient education. Patients are encouraged to understand their diagnoses, treatment options, expected benefits, potential risks, alternatives, and goals of treatment so they can participate actively in their healthcare.
