Dr. Hans C. Jeppesen, MBA, is a Board-Certified Internal Medicine physician, founder of Charter Health PC, and an experienced leader in Primary Care, Hospital Medicine, and Geriatric Medicine.

Dr. Jeppesen earned his medical degree in 2005 and completed his Internal Medicine residency in 2008 at the University of Minnesota. He has also been recognized as a Senior Fellow in Hospital Medicine by the Society of Hospital Medicine, reflecting his dedication to advancing patient care, medical quality, and healthcare innovation.

Since founding Charter Health PC in 2017, Dr. Jeppesen has remained committed to delivering personalized, patient-centered healthcare for adults throughout Massachusetts' North Shore, helping patients achieve better health through preventive care, chronic disease management, and coordinated medical services.
