Doctors told me what signs to look for at home and what to expect in case I need to be taken in again

This now doctor in another state put a false, stigmatizing condition in my health record that was not accurate several years back, as I just recently discovered at Ohio State Internists. She had no right to do this, was not my PCP but a resident doctor who took it upon herself to do this to a patient. I was never diagnosed with this condition by this resident, did not tell her I had this condition but was taking a certain medication for something else. I am working to have this removed but to have to jump through all these hoops all these years later to remove something I did not even know was listed on my current health conditions disgusts me and makes me very untrusting of health care practitioners.
