I think in this time of our medical system on the verge of collapse, Dr. Chalabi is thorough, extremely courteous, and in general exercises good practice. He will address every issue you mention, when the common practice of other doctors has become "one problem per visit". His nurse Karen is superlative, always returns calls, and works closely with the Dr. to get what you need. Overall they are exceptional in my mind.

Dr. Chalabi is very quick to refer to a specialist, which is good, but in my case failed to ensure the specialist had all necessary information. Specifically, Dr. Chalabi did not ensure that previous blood chemisty data was provided to the specialist. This can be very relevant when dealing with chronic issues such as cancer.
