I received an epidural steroid injection (ESI) from Dr Beerbower. After the procedure I have now learned that the drug that Dr Beerbower used for the ESI KENALOG-40 is NOT APPROVED BY THE FDA FOR EPIDURAL ADMINISTRATION. Dr Beerbower never informed me that he was injecting a drug into me on an off-label basis. Had I been properly and legally informed that the drug injected into me was NOT APPROVED by the FDA for epidural administration, I would have refused the injection. The package insert for KENALOG-40 from the Bristol-Myers Squibb website clearly states at the very top NOT FOR EPIDURAL USE. Why is a drug being injected into people epiduraly when the package insert clearly states NOT FOR EPIDURAL USE? At minimum, informed consent should be obtained from each patient. This is something that I never received. I have also learned that ESIs are extremely dangerous. Several people have even died from ESIs. Arachnoiditis is also another very serious complication that may result from ESIs.… I was never informed of any of these potentially very serious complications from ESIs. I would not recommend Dr Beerbower for any kind of invasive procedure due to him failure to give proper and legally required informed consent. For narcotic pain relievers he is probably OK.

My experience with Dr Baker , as a pain management patient has been nothing short of excellence. His superior knowledge , compassion and expertise is unsurpassed. Also Dr. Baker has performed numerous radiolological interpretations for myself and family, and we have NEVER had a negative experience or inaccurate report. His staff and facility are the best we have ever experienced . I highly recommend Dr. Baker to everyone.

Dr Baker purchased biologically contaminated injectable steroids from compounding pharmacies and injected them into patients at his former clinic Fairmont Diagnostic Center. Thanks for poisoning patients and ruining their lives.

Jack L Baker MD, Friendswood, TX, Lic. #J2489. An agreed order was entered on 8-9-97 suspending Dr. Baker's license; however, the suspension was stayed and he was placed on probation for seven years under various terms and conditions. Action due to intemperate use of alcohol or drugs.

Board and Jack L. Baker, M.D., entered into an Agreed Order publicly reprimanding Dr. Baker and requiring Dr. Baker to within one year pass the Medical Jurisprudence Exam within three attempts, within one year complete 24 hours of CME in ethics with a focus on billing practices. The Board found Dr. Baker engaged in unprofessional conduct, provided medically unnecessary services and submitted improper billing statements.

Dr. Baker is an excellent example of what and how a Dr. should care for their patients. Excellent at diagnosis and his advice is spot on.

I have known Dr. Jack Baker for a really long time, personally and professionally. I must say that he has a heart of gold and only wants what's best for his patients and everyone he comes in contact with. He is very knowledgeable in the Radiology field of study. Every procedure I've gotten, he has found the diagnosis, spot on. I would recommend my friends and family to Dr. Jack Baker because he's professional and loves what he does. He's a great doctor.
