Dr. Jantzen Sean Fowler, MD

Dr. Fowler was my assigned Doctor when I was in Rehab from a bad wreck. He is awesome. Has the best bedside manner. Gets to the root of the problem. Not afraid to correct staff when they are wrong or you for that matter.

This PCP was assigned to Medicaid patient, but this DR CANNOT be located ANYWHERE.

If it wasn't for Dr. Fowler, I very well could have had a heart attack. Very grateful to him for recognizing that I may have a problem and for sending me to a cardiologist.

Dr. Fowler always takes his time, listens to my concerns, conducts a thorough exam, answers my questions, and develops my treatment plan with my input. I could not ask for a better primary care physician.

Dr. Jantzen S. Fowler - I saw him on an ER Admit at Baptist Medical Center, Jacksonville, FL in 2013, and, he neglected my care, ignored my questions, and, ultimately him and another doctor dismissed me (vs. discharged me) in an unstable condition and I ultimately ended up in an ER and admitted to a different hospital less than a week later...needing an urgent left heart catheterization...without having been told at Baptist by Fowler or his colleague, Dr. Kelvin Bray. I find Baptist's Hospitalists, and, most of their doctors to be of poor quality, and, have done me harm throughout the years, on 3 separate ER admissions, as well as other times. I would not recommend him, nor this hospital group.
