Dr. Jason Patrick Collins, MD

This office is not run in a courteous manner. I made an appointment weeks in advance, yet I was called the day before asking for my medical records in order for the dr decide if he will see me. What's ridiculous is that a simple screening question wasnt asked when I made my appointment, which would have avoided me having to wait weeks of waiting for an appointment 9nly to being told less than 24 hours before my appointment the doctor declines to see me. Dr office with no common sense scheduleing an appointment.

I had a 130pm appointment today. I got stuck in traffic. Called office to let them know I was running a few minutes late due to I75 traffic. Arrived at front desk office 136pm (6 minutes late) Dr Collins said he had a very busy schedule today and I would need to reschedule. Over 65 and very disappointed!!!

run - don't walk to some other provider - every time I was there I was upcharged - lost my check - refused to acknowledge rx refills for my 3 month prescriptions - wouldn't give my lab results - kept wanting me to come back - has a pretty nurse is the only positive thing I can say but I would prefer she not call me on her call phone when I wouldn't return the auto dialer phone call - told them I had switched doctors and they still called - I told him I didn't want allergy shots and he insisted on talking about them and then charged me for a comprehensive visit when I was oniy there for 5 minutes. An office visit was like playing a board game and your times up - have to come back...

dr. collins prescribed the wrong meds causing a psychotic break. info was given to him regarding patients prior conditions but collins ignored it. could loose his md license over this but family chose not to pursue.

Can't see you even if you are very ill, drove 30 miles, because you were 4 minutes late. Of course you can wait on him for how ever long it takes him to get around to you. Not what a doctor should be.

I have seen Dr. Collins for 12 years. I have always been impressed with my care and treatment with every visit.

It appears that Dr. Collins will be happy to have you as a patient until you hit age 65; then he will not want to deal with Medicare paperwork and will ask you to find another doctor, which is what happened to us this morning.

Great experience overall. Dr. Collins has a great bedside manner and the office staff were very kind. I took my child for a well visit and it was our first time meeting Dr Collins. My husband and I plan to make appointments with him as well.

After moving here over 6 years ago, my husband and I had still not found a family doctor to go to. We rarely get sick, in our late twenties, maintain an active lifestyles and prefer a holistic approach to most illnesses. After a recent back injury, it forced my husband to finally see a physician. I searched for a provider from our insurance provider, and came across Dr. Collins. All I can say is WOW! After both my husband and I moved from our hometown's seeing the same doctors offices for all of our lives it was difficult to change and we would sometimes make the 2 hour drive back there just to see them. We are so happy we found Dr. Collins! Not only is Dr. Collins an incredible doctor, he truly cares. With his incredible bedside manner, medical expertise, and wonderful staff, I would recommend anyone to his practice. If you are looking for a family doctor, I would suggest that you hurry up and schedule an appointment, otherwise he might stop accepting new patients because his happy… existing patients may be all that he has time for! He is truly that good. Thank you Dr. Collins for being one of the few doctors out there that still cares about the work you do!

doc personable, seemed nice, but gave meds that were worse than his incorrect diagnosis. would not recommend him to my enemy.
