Dr. Jeffrey Evans Hazlewood, MD

I was greatly pleased and impressed with the high level and quality of care I received from Dr. Hazlewood and his staff! Dr. Hazlewood was thorough, kind, upfront, caring and professional. Staff was also professional, timely and accurately responsive. I had been living with daily pain 15 months post MVA, with only some progress, which had plateaued. I was frustrated by the pain and limitations it placed on my life and activity level. However, I am very health conscious and I am personally opposed to taking any pain meds. Dr. Hazlewood referred me to an excellent PT who helped me considerably. He also recommended a drug-free device to help manage and reduce pain. I was absolutely thrilled with these recommendations and have continued to make improvement! I recognize that my health is my responsibility and I am incredibly grateful for a physician I could work with who respects that approach and supports drug-free options. I highly and unreservedly recommend Dr. Jeffrey Hazlewood!

His staff are not cooperative. The doctor cares about his staff and not his patients.

I saw Dr Hazelwood a. Few years back he was friendly professional, I had a nice stay there my back was in bad shape he got me straightened out I didn't suffer excruciating pain any more, I could work good do all my life better I had a great experience, I liked him a lot, my therapy made a difference in my life, the meds would last me a long time, I'm so thankful that Dr.Hazelwood was there when I needed him I'm planning on making a visit soon, its been a while, but I know he helped me before , and I'm sure he can help me again,

Dr Hazelwood, and his entire staff, do an excellent job of putting patients interests first. They are helpful, courteous, and professional. He takes the time to listen patiently even though he has a very full schedule. I have never felt that he rushed through an examination with me. When I experienced a severe flare up in my lower back pain last year, and contemplated a second surgery, he went out of his way to help and advise me. He showed genuine concern and empathy for my situation. I was disappointed to see that some people rated him poorly, but I also understand that chronic pain can often negatively influence a persons attitude regarding a situation and cause them to say things they might not otherwise say.

Dr. Hazlelwood treat his workmen’s comp patients like animals not human Dr. Hazlewood does not care at all about you as a patient, his only concern is satisfying Worker Comp adjustors. He does not listen to you at all and does not care what you trying to tell him or explain to him how you are feeling or your symptoms he was rube to my husband cause he told him he was not helping my condition , it’s getting worse, every time I visits him, workmen comp want change my doctor so I can be close to home. I walk in the office in so much pain he sent be back home with no care, I end up going to my primary doctor end up having surge that workmen comp should had payed for. He has absolutely no compassion for you if you are in pain, he only cares about looking good for Workers Comp adjustors, this man is only practicing so he can put money in his pocket. I would not recommend him to treat anyone, one nurse needs to be fired for tamping with patient’s urine test does not know what she doing. I… told Dr. Hazelwood and workmen comp long rides cause me excruciating pain, he does not listen, spends a min. in the room with you and doesn’t act concern about your health. again Dr. Hazelwood and his nurse accurse me of failing a drug test twice, I always take my med. They tamper with my urine test half in my system and the other half not, I even witness one nurse pour it out of one cup into another said the cup wasn’t reading right. Workmen comp pays him to say what they want, sad playing with people health. This guy is a jerk. waste of time and money and I am still in excruciating pain, beware, beware of Dr. Hazlewood. I Don’t have anything personal against him, but when they starting lying on you making your medical record like bad to make them look good, no I have no more uses for Dr. Hazlewood or his nurses. The nurses always putting wrong notes in my medical record. I’m so disappointed. Dr. Hazlewood is more concerned about his practice than he is for his patients. It's his way or the highway Dr. Hazlewood looked at the paper work and would not listen to nothing that I said or anyone else. I'm still having the same trouble that I had. He did nothing for me, and he will do the same to you. It’s a crying shame This man needs his license pulled, and shouldn't be allowed to work on anyone horse. Never let workers comp send you to this man for pain management, avoid at all costs. You can hear him quack as he walks. My last words again, Dr. Hazlewood is unkind, uncaring, and ruthless. I see Dr. Hazelwood with work comp insurance due to an on the job injury. He and his staff are professional and treat me with respect and compassion. Being cautious in prescribing pain meds or running unnecessary tests is not a bad thing.

Not friendly doesn't want to help you thinks you're lying thinks you're just there for drugs which wasn't the case I wouldn't recommend him to my worst enemy

Do Not see this doctor!!! He is awful! Doesn't care about his patients, just working for the Insurance companies. Read his reviews, very poor. The positive ones are probably his friends. I RATE HIM IN THE NEGATIVES!!! Save yourself the waste of time, money and your health.

I have been treated by Dr. Hazlewood for the past several years for a work injury. He has always been very professional, kind and considerate. With more than 50 years experience in nursing and with many doctors during those 50+ years, I have found Dr. Hazlewood to be a physician who wants his patients to be as pain free as possible on the least amount of medication and help them remain as independent as possible and not rely on medications. He will give you what he feels you need but does not give an over abundance of pain medication as I have seen from other physicians in the past.
