About Dr. Kozicki
After completing undergraduate requirements at Michigan State University, Dr. Kozicki went on to receive his dental degree from the Indiana University School of Dentistry in 1989. He has been practicing general dentistry for over 15 years and has been at this location for over 10 years. Dr. Kozicki is a member of both the American Dental Association and the Michigan Dental Association.
Dr. Kozicki is married with one son and resides with his family in Rochester Hills. He enjoys outdoor activities including golf and sailing; attending local sporting events, and spending time with his family up north.
OFFICE POLICIES
TIME
If the office is running more than 10 minutes behind schedule, we will make every effort to contact you to let you know, as we understand that your time is valuable. Likewise if you will be late, we would appreciate a phone call, keeping in mind that if you are more than 10 minutes late, your appointment may have to be rescheduled.
A 24-hour notice is …required should you need to reschedule any appointments or you will be charged a $75.00 failed appointment fee.
CHILDREN
We encourage parents to let their children attempt to come back alone to the treatment rooms. This helps your child to develop self-confidence and provides them with the one-on-one attention he or she deserves from us. If your child has any difficulties, we will come to get you.
X RAYS
As a new patient, if you have had recent x-rays taken, either bring them with you, or we can obtain them from your previous dental office for you.
PAYMENT
Payment is expected the day of service. We accept cash, check, Visa, Master Card and Care Credit. For cash and check payments made the day of service, you will receive a 5% discount. Our policy for delinquent accounts requires that after 90 days, your account will be referred to our collection agency with an added fee for the collection process.
EMERGENCIES
If you experience a dental emergency, please call the office. Dr. Kozicki’s phone numbers are in the message should the office be closed. If we are closed for vacation, the phone number of the dentist covering for emergencies will be on the answering machine.
CELL PHONE POLICY
We ask that you please turn your cell phone OFF when you are taken back to the treatment room.
INSURANCE
Our office feels very strongly that every patient should be able to choose where they want to go for dental work. Therefore, we do not participate with dental plans that dictate who you must go to for your dental needs. We will bill your insurance if you make sure that we have the correct information.
