Julius Guiang

This review concerns Julius Guiang specifically, not vitals’s other providers or staff. I received psychiatric care from Julius through Headway / Keystone Adult & Adolescent Psychiatry, not through Med Harmony, but I am posting here because he currently practices with this group and prospective patients should know about my documented experience with him. During my care, I experienced serious problems with lab coordination, communication, and continuity of care. I was told that lab orders had been sent to Quest and that I did not need a paper requisition. When I arrived, Quest could not complete the testing because required diagnosis information was missing. Keystone later acknowledged in writing that the required diagnosis code had been inadvertently omitted and called it an oversight. I also requested a disability-related accommodation for important communications to be provided in writing. Keystone formally acknowledged that request on September 25, 2026. Later that same day, I… received a 30-day termination-of-care notice. I am not claiming the accommodation request caused the termination, but the timing concerned me. I already had an October 9 follow-up with Julius scheduled through Headway. Headway later confirmed the provider canceled it on September 27 with the reason “Client not the right fit,” and Headway could find no record that the standard cancellation email was generated. I also received changing written statements about the unresolved lab work. One letter initially said “No additional laboratory work” was required; after I asked about the already-ordered labs, a revised version changed that to “NO laboratory work,” without clearly explaining whether the original orders were corrected, canceled, or deemed unnecessary. Headway documented my concerns as a formal grievance. Based on my experience, I would not choose Julius as my provider again.

My experience with Julius Guiang / Keystone Adult & Adolescent Psychiatry involved repeated problems with communication, laboratory coordination, medication-related administration, and continuity of care. Despite repeated follow-up, I never received a clear resolution showing that the existing orders had actually been corrected and made usable. I also submitted a disability-related request for important communications to be provided in writing. Keystone formally acknowledged that request on September 25. Later that same day, I received a 30-day… notice terminating my psychiatric care, with the stated explanation that my healthcare needs were no longer appropriately suited to the structure and limitations of the practice's telehealth model. I am not claiming that my accommodation request caused the termination, because I cannot establish their motive, but the timing was concerning to me. There were additional continuity-of-care problems after termination was announced. I already had an October 9 follow-up appointment scheduled through Headway. Headway later confirmed to me in writing that the appointment was canceled by the provider on September 27, with the reason selected in Headway's system being “Client not the right fit.” Headway also told me it could find no record that the standard automated cancellation email was generated, so I was not properly notified through the normal process. I was also concerned by changing written explanations about my outstanding lab work. An initial transition letter said “No additional laboratory work” was required. After I specifically asked what happened to the laboratory work that had already been ordered but could not be completed because of the acknowledged ordering error, Keystone sent a revised version changing that language to “NO laboratory work.” I still did not receive a straightforward explanation of whether the original orders had been corrected, formally canceled, or later determined to be clinically unnecessary. Headway ultimately documented my concerns as a formal grievance and stated that the matter would go through its internal clinical review process. I eventually had to arrange replacement psychiatric care and records transfer during the termination period myself. Based on my experience, I would not return to this provider. Patients who depend on reliable prescription/lab coordination, clear written communication, or careful continuity-of-care planning may want to keep copies of everything, verify orders directly, and document important communications in writing.
