Dr Seen at River Bend ER in Eugene. Failed to diagnose concussion despite being told I was light headed, dizzy, had a headache and was nauseated.Reled on concussed crying patient to tell him what happened, when spouse who found me face down motionless was right there and was nor asked what he saw. Did not order imaging despite our requesting it. Did not do Glascow Coma Score or Sensory mtor testing. Did not call in specialist and delayed treatment by giving a specialist referral for a Dr who was not available for 3 days. no aftercare instruction for concussion. Patient vomited severly later at home. Concussion diagnosed 10 days later by my primary care Dr. who took over after the refrral specialist also made abnormal findings and failed to oreder imaging studies. Missed 7 day window for closed reduction because of this. Remained injured for 9 full months. Required follow up care with facial reconstructive surgeons and had to have nose reconstructed 9 months later and air way rebuilt… with ear cartilage grafts.

Refused service on pain in arm and shoulder with bp of180/120
