Dr. Nina Marie Barnes, PhD

I was directed to the Dr for a review. I arrived at a residential dwelling with a side room dedicated to patients. The Dr then asked me to sign a one page release, which is generally a routine enough process. However the release asked me to acknowledge that my medical info including her review was subject to "unauthorized release" and asked that I waive rights to privacy protection under federal and state law. I told the Dr Barnes I was uncomfortable with signing a release for unauthorized release and waiving my privacy rights under state and federal law. She advised I was reading it incorrectly, so I contacted a lawyer, who I was able to get immediately on the phone and I read the phrase of concern to her. The attorney also indicated I should not sign the form under any circumstances. She did make suggestions on how a few simple changes would make the form bopth legal and palatable. When I suggested we make those changes the Dr indicated I was too legalistic, refused to… complete the appointment and asked me to leave her office byt stating "it is time for you to go." Note that I was in the office about 12 minutes from when I turned off my vehicle to when I arrived back at the vehicle. I would be EXTREMELY careful about the paperwork signed with this provider.
