Dr. Pejman Mokhtar

Mt insurance card shows the Dr. As my primary dentist. On 6 10 2019 jesica called United Concordia to verify my Insurance. She failed to tell me they were no longer my Dentist but instead played along. When I asked how much my crown copay was going to be he told me the crown was 1800.00 dollars and my part was 1100.00. Went to endodontist today and they said I was referred as a cash patient. They called my insurance. Ken at united concordia verified that my crown co pay was 30.00 and that he had spoken with jessica on 6 -10-19 and told them that they were no longer my dentist. Jesica never told me. The also tried to defraud my 33 year old son with a 30,000 dollor estimate to fix his teeth and never told him they were no longer with United Concordia. Man is a crook. I wonder if he actually has a license.
