Rachael Julson moved, seconded by Timothy Szczykutowicz, to find that grounds exist to deny the licensed radiographer application of Randolph Magone, and to offer him a limited license with the following conditions: AODA assessment prior to beginning practice within sixty (60) days of the order; testing and reporting commensurate with a 5 year impairment (52 tests per year with 1 hair test, work reports), compliance with the terms of any active orders from other states. Reason for Denial: Wis. Stats. §§ 462.07(2)(b) and (e) and Wis. Admin. Code §§ Rad 2.05 and Rad 6.01(3) and (15)(license encumbrance, disciplinary action in another state, actual or potential inability to practice with reasonable skill and safety due to alcohol or drugs). Motion carried unanimously
