At intake, I gave Mr. Ronald S. Monson an official decision by an Administrative Law Judge at the Social Security Administration confirming my mental disability of "bipolar disorder." However, he immediately set the document aside unread. Instead, he asked only yes and no questions, clearly neglecting to undertake a familiar detailed diagnostic evaluation expected at intake. In addition, I showed him all my medication vials, but he retorted rudely that he would "call the shots." When I later requested his record, he contended that I have a "personality disorder" without substantiation. Moreover, not only did he fail to record my prescriptions in the medical chart, but he also never ordered refills at my pharmacy. I had to remind him twice by email and then finally by phone. We made a second appointment, which he later forgot to record in his calendar. Abrupt, rushed, and careless. Exam lasted only 15 minutes but billed to Medicare for 45 minutes.
