I had a number of difficult items to discuss. Because he had reviewed my records in my MyChart accounts, Dr. Yarnall also had these items on his agenda for this visit. I needed to discuss readiness for hospice. He mentioned that he had seen in a visit summary by my pulmonologist that I had begun a conversation about this, making the transition to talking about it very easy and natural.
