I am leaving this review based on my experience as a patient seeking continuity of care. My concern was not a disagreement over treatment preferences, but a lack of thorough chart review and individualized, evidence-based care. During my interaction with Shawna Vajda, it was clear that my medical history had not been fully reviewed, which led to conclusions that did not accurately reflect my documented care plan or clinical timeline. Attempts to clarify this were not meaningfully addressed. Additionally, changes to an established care plan were made without an in-person visit or physical evaluation, which further limited the opportunity for appropriate assessment and shared decision-making. As a patient with a complex medical history, it is essential that providers review records in full and base decisions on the complete clinical picture. Unfortunately, that standard was not met in this case. I did not feel heard or adequately advocated for, which significantly impacted my… confidence in the care provided. I hope this feedback is used to improve patient care and provider engagement moving forward.
