Dr. Mohammad Sohail Alam, MD

Dr did not read patient chart. Dr. Discharged patient with long acting insulin meds for a new diagnosed patient but did not include any plan for short acting insulin medication despite knowing the patient was diagnosed 24 hours prior. Dr failed to offer advise or even schedule out patient care for endocrinologist. Dr did not even know patient suffered from intercranial hyper tension Dr did not know patient had been in the hospital less than a year with minor heart attack and had stent placed. Dr assumed the patient knew they were a diabetic. Dr told patient to discontinue medication to treat hypertension in discharge orders. Dr had poor communication skills over looked and provided incorrect diabedies information to the patient. I would not recommend this doctor to anyone with complex multilevel systematic health issue's. Dr left the patient and did not even stay for follow up questions. This should have been a total disaster except we have family with the disease and had… some education on the subject which is how we knew what the doctor got right and wrong. I fail to see how a doctor can treat a patient if they do not read the basic case file information.

Sometimes appeared to be disorganized - couldn't immediately find lab tests in patient file
