Wanna-be tough guy. Made an appointment two weeks in advance then was told they had no record of the appointment. Gave me a new one and we got there early to do the forms. One form was troubling, their "Office Policies", it said they had a fine for not cancelling your appointment more than 24 hours in advance, the fine was $50. It also said the patient was responsible for lawyers the dentist would use in going after a past due bill. This last part was crazy in that they expected to be paid on the day of service. I printed my name on the form, but didn't sign. I told the receptionist about this when returning the forms. She said she needed to talk to the dentist and left. He came out looking confrontational and asked me "What can I do for you?, as if the receptionist didn't let him know the situation. He finally said that the forms needed to be filled out before he would see me. I said that I would pass on that and that I would take the forms home to shred since my info was on them. He… started to tear the forms up and then tried to hand them to me. At that point I said "You get rid of them" and left.
