POLICE REPORT FILED (following is an evidential information to the Public Police Report filed) A mother made an appointment with Peninsula Pediatrics of West Florida (PPWF) for her child to receive a missing shot and convert an out-of-state vaccination record to a FL standard in order to attend FL school. Due to nurse Lesley Mallett gross incompetence, mother left PPWF office without completing an appointment. Within ½ hr PPWF office called FL Abuse Hotline initiating a Child Protective Services investigation against mother with allegations ranging from mother appearing not to be an actual mother to a child to a medical neglect charges. The investigation was closed with no indicators of any credible evidence to support the allegations. In turn, mother filed a police report with Sheriff’s Office initiating criminal investigation against PPWF for false reporting to harass and embarrass her. Investigation was moved to the Inactive Status due to no additional evidence at the time. In… addition, the PPWF office attempt to charge mother’s insurance $470 for a simple wellness visit (claim denied). Mother retained an attorney after PPWF failure to provide medical records only to receive 2 page incomplete record vs. 9 pages provided to the detective during an investigation. Now you can make an informed decision if you would like to make this office your child’s medical provider.

Excellent pediatrician. Kind, patient and attentive to child and parent. Very knowledgeable in his field and very helpful with references outsourced with follow through. Bedside manner is excellent. A genuine concern for patient and outcome.
