Gingivitis Treatment

Recently, I have had many patients ask about my suggestions to treat gingivitis.  I even received a review questioning my treatment as “not needed”.  The first thing to recognize is the gingivitis is an oral disease that is caused by bacteria.  The severity of gingivitis can range from mild to severe when it can be seen during a routine oral health maintenance examination.  The second thing to recognize is  that my treatment is based solely on your diagnosis.  I will admit that I am more proactive in my treatment.  However, I have seen dramatic results to cure this disease from my recommendations.  I know that when I reverse gingivitis and it will not progress to a more advanced form of gum disease – periodontitis.  Furthermore, I have seen, firsthand, the affects of gum disease has had on a close family member.  If, I can stop that in my patients, then I am doing my job.
What is my treatment of gingivitis?  Generally, it has a number of parameters that influence my regimen.  These include the severity, the patient’s dental history, the patient’s oral hygiene, and other medical history.  These parameters influence and modify the general treatment recommendations.  Because their are many unique situations, I will focus on the generalities of the typical treatment plan.  Typically, if you have not been to the dentist for more than a year, I find most people have moderate to severe gingivitis.  I will say only 5 out of 100 have mild gingivitis.  I believe that all people should receive a cleaning every 4 months.  The reasoning is that the bad bacteria begins to manifest in the pocket between the gums and teeth at this time.  However, because insurance companies pay for cleanings 2 times a year, cleanings every 6 months has become the norm, which is insurance driven and not medically driven.  Based on this information, you can see that the bad bacteria has been doing damage for at least 8 months.  Clearly, the more time that passes without a proper professional cleaning, the more time the bacteria has to inflect more damage.  In order to ensure that I reverse gingivitis, I recommend 2 professional cleanings and a strong antimicrobial mouth rinse.  After the first cleaning, the second cleaning will be given after a two week regimen of the mouth rinse.  This will wipe out the gingivitis in most cases.  When it is reversed, I will return the patient to the typical 6 month cycle.
Does this treatment work? Absolutely!  Is it needed?  Yes!  Do you have to accept the recommendation?  No.  If it is not accepted, it is because their is not a clear understanding of how gingivitis is a gateway to the advance gum disease that destroys and melts away jaw bone or because dental insurance will not pay for second cleaning within the two week time frame.  I do not know the exact amount of our cleanings, but lets say they are about $70 +/- $10.  I believe that this minimal investment to ensure that gingivitis does not progress to a more severe form of gum disease.  If you think $70 is a price not worth paying, imagine what it will cost to treat a more severe form.  I am not even including the long term costs of your total health.  One way to look at this is by asking you a simple question, “If I told you that your elbow has a strong possibility to dissolve, would you want to prevent this from happening?” I do not know 1 person who would want their elbow to dissolve or not prevent it from happening.  This is what could happen to your jaw bone and I am offering you a treatment to prevent this.  Let me address the review about prescribing a mouth rinse and their view that it is “not neededâ
