BEWARE!!! I should have been suspect when they had appointments available the next day. Hindsight is 20-20. I submitted an online request and waited 10 days for a call (or email). I finally called. Their office staff informed me that this location doesn't get any of the information - this seems both absurd and inefficient. After filling out 8 -10 pages of documentation (which is customary for new patients) I was informed that I would not be seen. They were refusing to see me because I declined the arbitration clause in their forms. First, I am 100% within my rights to decline arbitration. Second, I believe in the justice system and IF there was ever a problem, I would want a court to be the deciding party (not a potentially biased single person). Third, if a doctor had confidence in their skills and staff, this would be a non-issue. Only someone who has practiced in a way that breeds claims of malpractice wouldn't allow a patient to decline arbitration.
