Momentum Medicine Plus

I saw this Dr for my SSA required physical in December. I got denied my benefits in January, and the denial was a direct result of the report provided by Dr Cornejo. The report from beginning to end has been falsified to ensure my denial of SSDI benefits. I recorded this visit because I have short term memory loss, and my husband was barred from going back with me. I have a 42 min long audio recording of my appointment from beginning to end PROVING he falsified my medical records. All of this has been turned over to all state government and federal agencies and the medical board for review with the complaints I filed. There is IRREFUTABLE EVIDENCE he committed perjury and doctored my medical records to guarantee my benefit denial. DO NOT ALLOW SSA OR THE VA TO SCHEDULE YOU WITH THIS DR. IF YOU MUST SEE HIM, DONT LET THEM TELL YOU THAT YOU CANT AUDIO RECORD. RECORD THE ENTIRE INTERACTION BECAUSE THIS MAN WILL LIE THROUGH HIS TEETH TO GET THE KICKBACKS GENERATED BY HIS SSDI DENIALS. He… even lied and said I didn't use a cane when you can hear him trip over it in the audio recording, and me apologize for it being in his way. He lied about everything besides my name and date of birth. It was ALL a lie.
