Immediately after We noticed this We contacted the Clinic and instead of fixing the falsified clinical summary Jennifer Ann Coddington with the help of others instead contacted DCS to allege that Our[ the parents ] behavior was erratic . Jennifer Ann Coddington is not license to practice behavioral health in the State of Indiana and used their position as a CPNP to make assumptions about us when We first met this provider . Our son's appointment was recorded by us and where in that recording it affirms this provider lied to DCS to cover up the fact the provider falsified Our son's medical information in contradiction to his prior medical history and used DCS to retaliate on us to hide the fact of the falsified… information shown on the Clinical Summary . The Clinic and provider where made aware of the concerns of missing known allergies of Our son and other information that was falsely listed and asked to correct their errors and instead an hr later they called DCS and made a false report. This provider has clearly and affirmatively shown that valid concerns for Our son may be ignored and neglected by her and the provider may conceal and keep the falsified information in Our sons records instead of correcting the information showing further dereliction and a lack of care for the wellbeing of Our son.
