I am writing to file a formal complaint regarding the unacceptable care my father received under the supervision of Nurse Practitioner Glenda Hall at WellMed. The mismanagement and errors in his treatment have caused significant emotional distress for both him and our family, and raise serious concerns about the quality of care at this facility. Key issues include: Incorrect Diagnosis My father was inaccurately diagnosed with diabetes—a condition he has never had. This false diagnosis was entered into his medical record and became the basis for further inappropriate action and treatment. Wrong Medications Following several hospital visits, my father’s medications were changed. Despite this, WellMed continued prescribing medications that were no longer appropriate. This not only contradicted hospital instructions but placed his health at serious risk. Unwarranted APS Report Based on their erroneous records, WellMed contacted Adult Protective Services (APS), falsely stating that my… father had uncontrolled diabetes due to improper medication management. This accusation was completely unfounded and created unnecessary and traumatic involvement by APS, causing my father anxiety and stress. Lack of Accountability and Professionalism When confronted with mistakes, the response from the office has been dismissive and unhelpful. There has been no meaningful attempt to correct the records, apologize, or ensure this kind of malpractice does not continue. This level of incompetence is not only unacceptable—it is dangerous. I urge you to investigate this matter thoroughly. I am requesting a formal correction of all inaccuracies in my father’s medical record, and an internal review of the staff you choose. I am changing Doctors due to years of frustration I love Dr Garcia, but the business Dr Hearn built went bad when y’all took over. All you want to do is see patients and bill, bill, bill making in-necessary appointments and giving un-necessary test.
