This review is not to discredit, but to inform, protect, and drive improvement—so families can make better decisions, and physicians uphold their duty with accountability and compassion. 1)Prognosis and Discharge Planning Dr. Ahmed Shaikh was involved in my loved one’s six-week ICU stay at Mount Sinai West. In our experience, there appeared to be early assumptions about the expected trajectory of her condition. After showing modest improvement, she was transferred out of intensive care without a comprehensive care plan or evaluation of her ongoing vulnerabilities. 2)Readmission and Follow-Up Concerns Within days, her condition worsened, and she was readmitted. This raised concerns for our family about whether her transition from ICU had been appropriately timed. 3)Team Coordination and Oversight Dr. Shaikh’s role as a… critical care physician included guiding multidisciplinary collaboration. We observed lapses in coordination, including unclear follow-through across departments during critical moments. 4)Clinical Education vs. Direct Care In an academic setting, training responsibilities are important. However, in our case, oversight seemed to emphasize education more than consistent, personalized care—something we found concerning for a patient with complex needs. **Suggestions for Improvement** – Carefully assess readiness before transferring patients from critical care settings. – Reinforce real-time team coordination, especially for those with complex conditions. – Balance medical education with strong oversight and timely, direct care. – Treat elderly patients with the same focus and engagement afforded to all. In our experience, greater attentiveness and stronger integration across care teams would have led to better continuity. Patients and families benefit most when care is proactive, collaborative, and responsive at every step.
