Where does one begin with the nescience of this provider. For starters this provider never formally or informally introduced herself upon first contact. I thought she was a student for majority of my introductory visit. I have been a patient with chronic illness for 27yrs and lost previous PCP due to pandemic. I am no stranger to medical community here or nationally. My office notes that were taken are incorrect. Wrong specialists names were entered. Incorrect medical terminology was recorded. Provider incorrectly coded my visits. I submitted a request to amend the records it was refused. I checked MyChart immediately after receiving this info via mail and the notes are now changed with no record of her error. Ask for your AVS and READ IT before you leave the building to avoid medical errors on your record as this is obviously an inept and callous provider.
