My role extends to coordinating care for chronic diseases, aiming to minimize hospital readmission and enhance patient outcomes. I'm also well-versed in the nuances of healthcare coding and reimbursement processes, ensuring that patients receive the appropriate resources for their healthcare needs. Prior to my current endeavors, I crafted individualized care plans for hospice patients in their homes, ensuring comfort and comprehensive support during end-of-life care.
