June 25 Visit: I visited this practice seeking routine medical care and assistance with workplace accommodation paperwork. During my appointment, the interaction with the physician was courteous and professional. However, after reviewing my medical record and the documentation prepared for my employer, I identified statements that I strongly dispute and believe require immediate review, correction, or clarification. This is urgent because medical documentation does not remain isolated inside a chart. It can be relied upon by CPS, courts, current and future employers, insurers, healthcare providers, and other agencies. References to mental health concerns—if inaccurate, unsupported, or poorly explained—can seriously affect a person's employment, custody rights, access to services, credibility, reputation, and ability to operate as an independent adult. I am a devout Muslim. My prayers, fasting, and religious discipline are sincere acts of worship. At times, I pray and fast beyond… what many may consider ordinary, but that does not make those practices evidence of mental illness or impairment. I am deeply concerned that my religious practices are being framed as tools to alleviate mental health or other issues rather than being recognized as sincerely held religious obligations and devotion. Since I began organizing regular Friday prayer services in Uptown Charlotte, I have observed a troubling pattern in which references to mental health issues have appeared in records and proceedings in ways that I strongly dispute. These labels are extremely damaging when unsupported, especially when they affect employment, custody, religious freedom, and access to services. I also question whether this provider was qualified, within the scope of this visit, to make or imply mental-health-related assessments in documentation provided to my employer, especially when the visit was for routine care and workplace accommodation paperwork. I have formally requested that the provider identify the basis for the disputed entries and correct or clarify the record immediately if the statements are unsupported. If the provider's assessment is shown to be accurate and supported by the underlying records, I will update this review accordingly. Patients deserve medical records that are accurate, objective, carefully worded, and supported by evidence—especially when those records may affect fundamental rights, religious freedom, parental rights, employment, and access to services.
