Dr. Jeffrey Alan Kozak, DO

Excellent physician, very knowledgable, got me in same-day, answered all my questions, staff was wonderful, would highly recommend.

I have had the privilege of seeing Dr. Kozak for a hurt shoulder. He listened to my concern and came up with a plan of care that was comfortable for me. He was not pushy, which I am appreciative of, allowing me to try conservative treatment first. Unfortunately, in my case, I did have to return for trigger point injections. He was very patient with me due to my fear of needles. He explained the procedure. He made me feel comfortable. The injections helped and I am grateful that I found Dr. Kozak.

After miss diagnose and very painful physical therapy for herniated disc, i saw doctor Kozak. He immediately corrected the diagnose to cracked disc and recommended alternative treatment and not a surgery. Explained the risks and complexity of a surgery in my situation and possibility of ending up in a wheel chair. It has been 18 moths of careful and patient exercises, and I'm happy to report I'm 99% pain free. Even my S1 nerve is coming to life again. No more "floppy foot" syndrome. Thank you doctor Kozak!
