I had an appt. for my daughter to get her seasonal booster vaccine. I went in and the receptionist told me I needed to fill out a form. I had my daughter in hand and no clip board was provided. I asked the receptionist why I needed to fill out the form. Vanessa Villanueva stated it was because the nurse was administering the vaccine and not the doctor. She stated it was the form was for allergies. Very basic questions. Are you allergic to chicken eggs? no, latex, sick in the last 24 hours... No mention that the form was for the H1N1 shot. I go in my daughter is vaccinated I leave and get a phone call 1 hour later from Vanessa asking me if I wanted to set up an appt. for the seasonal booster? I was just there for that. No, you were given the H1N1!!! What???? I asked you why I needed to fill out the form and you stated for allergies. Her response was "It was at the top of the form"!!!! I had a copy faxed to me and in 10 size font is states pediatric H1N1 Influenza with a sticker covering… part of the title with my daughters medical record # DOB and so on. I was told by the Jennie O. that I must have been given the form because several other patients were there for the H1N1. I asked to speak with Vanessa's supervisor Kathie. Given runaround!!

I took my little one to her two times in a row for the same problem and she did not do anything . her answer to me there is always emergency room, yeah right and there is always another Doctors around so i will not be stuck with her .
