We are located just five minutes north of Pembroke Pines mall. At the south east corner of Hiatus Road and Taft Street in Pembroke Pines, Florida.
Insurance
We accept all PPO Insurances. We are Participating provider with Cigna, Delta Dental, Humana/Comp Benefits, Dente-max Network and Memorial Healthcare.
Understand your Insurance Benefits:
Benefits are not determined by our office. We do not have a contract with your Insurance. The percentage paid is usually determined for how much you or your employer has paid for …coverage or the type of contract set up with the Insurance Company. However, as a courtesy to our patients, we are happy to accept majordental insurance provided we have a signed claim form and benefit booklet on your initial visit.
You will be responsible for all services not covered by your insurance, as Deductibles and co-payments. It is your responsibility to understand your benefits and limitations. These co-payments are due by the time of treatment.
If there any changes in your Employer Insurance Policy, please let us know as soon as possible. Your cooperation is appreciated and will result in receiving high quality dental care at the lowest possible cost.
Payment Options
In an effort to help make dental care for your children more affordable in these times of escalating health care costs, we accept: Cash, Master Card, Visa, Discover and American Express card. Fees incurred are due in full when services are rendered.
Appointments Scheduling
Regular visits are by appointment only. Your scheduled appointment time has been reserved specifically for you. We request 24 hours notice if you need to cancel or reschedule your appointment. This way, we may reschedule at a better time for you and allow us to schedule other patients. A fee may be charged if a patient does not show-up for an appointment without sufficient notice.
Morning Appointments
Children under 6 Years old should be seen in the morning. They are less tired, more energetic and cooperative to feel more comfortable.
For the same reason, school children with treatment work under nitrous oxide be treated in the morning.
Dental appointments are an EXCUSED ABSENCE. We will be happy to provide you with a Dental Excuse for your child’s School. Missing School can be kept to a minimum when regular dental care is continued.
Dental Emergencies
If do you have a dental emergency at our regular hours, please contact us to accommodate your child the same day. Please do not come directly to the office. By calling us, you will allow us to create a time to see you.
Dental emergencies after business hours, if your child is a patient of record, can be handled by calling our office and contacting the emergency line to speak to a dentist.
