I made an appointment and gave my insurance information on the phone. On the phone I told them I have both Medicare and a supplemental insurance and they participate in both. I filled out all the forms online but left out my social security number which I don't give out to anyone because of the possibility of identity theft. When I arrived at the office I was told the doctor would not see me without my social security number because in case I didn't pay my bill they needed that to send me to collection. They already had my insurance cards and a copy of my license. I pay my bills plus they participate in both of my insurances and I was willing to pay my copay upfront. I was told that my only alternative was to pay out of pocket for everything with no insurance. I left the office. This is a warning to anyone who feels like me that having my SS# in the same place as my license and other identifying information is a recipe for identity theft. As an aside a few years ago a friend of… my had that exact thing happen to her via a doctor's office.
