Dr. Norberto Sibayan Perez, DO

Dr. Norberto Perez was assigned to my mother after she had heart surgery at Banner Hospital in Mesa AZ. She suffered a major stroke, contracted CIDIFF and was transferred to Springdale Village nursing home in Mesa to recover. She was then under the care of Dr, Perez who visited her weekly. We were never able to call Dr. Perez and left many messages for him. He never returned our calls. There were many questions regarding my mother's progress and her care. After she was released prematurely on 3/1 (and my father only given 2 days notice - she was incontinent, unable to walk and needed 24/7 hour care) we flew her home to WI with a nurse. She died one month later. When she had arrived in WI she had already lost 25% of her body weight. I would NOT recommend Perez to anyone under any circumstances. I feel his lack of care and inaction contributed to my mother's condition and eventual death.
