I was a patient of Dr. Hazelbaker's for yrs. He was always professional, kind and understanding. I have been searching for him with no luck! I want him to know that I stopped smoking and doing medication for a couple of yrs. now. Listened to his good advice. However, I do have other health issues that need addressing, and if anyone out there knows where he is practicing, please post it on the internet! Thank you!

I'm a big fan - I've only seen him a few times but he's very chill - more about maximizing and enjoying life than most docs I've had.

I was under Dr. Hazelbaker's care for a number of years until 2012 when I went to an appointment and he was not there, and never would be back. I appreciated the way he practiced without fear of reprisal and willing to perform simple procedures efficiently and quickly. He was great at diagnoses and explained his rationale clearly and concisely. I have not found another doctor and have felt truly abandoned by Mr. Hazelbaker. I have been denied the prescription that he recommended and so have many sleepless nights. Frankly, I am not concerned about my health care any longer, so will just wait for emergencies to arise. Very sad.

hes an awesome popular local doctor who is down to earth & caring. he spends quality time with each patient. sometimes difficult to get a prompt appt due to the above facts so it should be made 2-4wks ahead of time. been a patient of his for 5 yrs now, hes great!

I've seen Dr Hazelbaker 4 times and everytime I had to wait over an hour. The first time they put me in a room and said: The Dr. will be right back. I waited 45 minutes. The rest of the times they put me on a room but they didn't even said how long it would take. The 3 times took over an hour, the last time it was 1:40 minutes, so I said enough!The Dr. himself is not a friendly guy at all, it seems like he is not even happy being there.
