As your Primary Care / Family Practice physician, our goal is to provide you and your family with continuing, complete, preventive care and partner with you in your journey toward good health and wellness. We recommend annual well-visits and physicals so we can detect changes in your health from the previous year, know which immunizations you and your family need, or help you manage chronic conditions such as diabetes or high blood pressure. When life happens, our caring staff and respected physicians are here to treat your school-aged child with strep throat, your youngest who has a foreign object in her nose, and your elderly mother whose foot is swollen and red from recurring problems with blood clots and arthritis. Our family practice physicians specialize in treating all of the above and continue to explore the best, most effective ways to do so.

I HAVE HAD DR. WILBERT JACOBS FOR ABOUT 3YRS AND HE IS THORO IN HIS EXAMINATIONS AND SIT AND EXPLAINS WHAT THE SITUATION IS. NEVER FEEL RUSHED HE WAS ATTITUDE FREE NO WORRIES IT WAS ALL ABOUT THE PATIENT JACKSONVILLE HAVE SOME DOCTORS THAT DONT CARE ABOUT BECAUSE THEY ARE BEING OVER BOOKED. THE DOCTORS OFFICE NEVER LET YOU SIT NO MORE THAN 10 MIN. UNTIL YOU ARE IN THE BACK. I AM GOING TO MISS HIM WHERE IS HE PRACTICING? I PRAY TO GOD HE BLESS ME WITH A DOCTOR LIKE DOCTOR JACOBS I WOULD GIVE HIM A 5 STARS OVER. THANK YOU SO MUCH FOR CARING FOR ME. MS. YVONNE D. JACKSON

I have been a patient of Dr. Jacobs for the last 5 years. Never once have I felt rushed or like he was annoyed with anything I had to say. He is super down to earth. I am very upset that he is no longer with Family Practice Associates. I will definitely miss him.

Dr Jacobs is very compassionate and caring. He not only explains EVERYTHING to you in detail but is very, very knowledgeable and kind. I trust him with me and my family's healthcare.

Dr. Jacobs and the staff are so friendly and caring. It is so refreshing to have a doctor that actually listens to you. I absolutely trust him with my health care. I couldn't be more pleased!

I met Dr. Jacobs just over 2 years ago, and he responded very compassionately and immediately to the issue I was experiencing at that time. I had reason to see him this week, and I was able to get in on the same day and found him and the staff to be very kind and responsive to my needs. Dr. Jacobs is very professional, very pleasant, and willing to help. I highly recommend him and the staff in this office!

When I first got an hmo I was slightly worried about finding a new dr. As soon a i saw Dr Jacobs I felt so received. He is so down to earth. Treats me with kindness. Never rushes me. Answers all my questions..Explains things in a manner I can understand. His entire staff are all courteous and professional. I have always been able to get an appointment within a reasonable time frame. According to urgency. I would highly recommend Dr.Jacobs to anyone looking for a Dr.
