I am submitting this feedback to document serious concerns regarding communication, documentation, and quality of care during my interactions with this physician. Over the course of several visits in 2025, my appointments were primarily for referrals and follow-up issues. While I did not have an opportunity to meaningfully evaluate this physician’s overall clinical expertise, my experience raised significant concerns regarding professionalism, documentation practices, and patient-centered care. At my first visit in April 2025, which was for a referral and when I was not experiencing pain, the physician requested to audio record our visit on his personal cell phone. I declined. This request made me uncomfortable and did not feel appropriate in a medical setting. On multiple occasions, I was encouraged to communicate about medical matters via standard text messages rather than through the secure patient portal. I prefer and expect medical communication to be handled through… secure, documented systems so that my medical record is accurate and complete. A more serious concern occurred at my visit on December 5, 2025, approximately one month after my hip replacement surgery. At that visit, I presented with weakness and also specifically complained of ear discomfort and chronic sinusitis symptoms. The physician briefly examined my ears, stated they were clear, and told me he did not believe I needed to see an ENT specialist. Over the following week, my symptoms worsened, including increased mucus, ear pressure, and congestion. When I reviewed my After Visit Notes, I discovered that my ear and sinus complaints were not documented at all. There was no mention of my reported symptoms in the medical record. Because my symptoms continued to worsen, I sent a message requesting a referral to a specialist and noted my history of chronic sinusitis. Instead of addressing this in writing, I was told that the physician wanted to see me again in person. I ultimately sought care from another physician, who prescribed antibiotics and provided the appropriate referral. The lack of documentation of my reported symptoms, combined with the initial dismissal of my concerns, is particularly troubling. Accurate documentation is critical for continuity of care and patient safety. There was also an earlier instance in July when I injured my shoulder and was informed that a referral was required. Despite experiencing pain, I was told to wait approximately two weeks for an in-person appointment, and a brief phone consultation was not offered to help address the issue in a timely manner. As a result, I sought evaluation and diagnostic imaging, including MRI, outside of Kaiser. This reflects a delay in timely access to care within the system and resulted in care and testing being obtained outside of Kaiser. In addition, I experienced ongoing difficulty getting clear documentation errors and outdated problem list entries corrected. Even for administrative and documentation matters, I was repeatedly directed to schedule in-person appointments rather than having these issues addressed in writing. Overall, my experience reflected a pattern of: • requests to use personal devices and non-secure communication methods • inadequate or missing documentation of patient complaints • reluctance to address administrative and documentation issues without requiring in-person visits • delays in addressing acute concerns when timely guidance or referrals were appropriate These issues raise concerns about communication standards, documentation accuracy, and respect for patient time and safety. For these reasons, I have decided to transfer my care to another provider. I am sharing this feedback so that these practices can be reviewed and improved for the benefit of other patients.
