I specifically requested a cognitive evaluation and was assured this would be done. It was not. No recognized cognitive evaluation tool was used, no meaningful questions that could test short term memory or ability to assess and analyze new information were asked, and no documentation was provided. I would not recommend this doctor to anyone.
