Dr. Pedro Choca, MD

This guy is a joke. I saw him twice for anxiety over my job. He acted like he was filling out FMLA paperwork to help me. The first time he sent the document to my agency, he didn't sign them. Who thinks FMLA documents will be approved without a doctor's signature? I contacted him directly and asked him why he didn't sign them. He said he wasn't a doctor! I contacted him and Dr. Shealey, the primary who recommended him and who won't fill out FMLA paperwork. Apparently that's not in her job description. I sent them both copies of the form the agency sent me with huge arrows of what needed to be filled out. Not only did he not sign it but he didn't even check any of the boxes that needed to be checked. I was totally stressed out and took several days off of work to de-stress. While I was out, I got a message from Dr. Choca's office advising me that the paperwork had been resubmitted. Except I was contacted by the FMLA agency that it still wasn't completed correctly. He… still didn't complete the body of the form. Also, neither of my visits were helpful. He told me the same thing for both appointments ... do breathing exercises and think positive thoughts. He pretty much said the exact same thing in both appointments. And I have to pay out of network bills for these appointments. What a joke. Steer clear of this guy!
