Dr. Jeffrey Alan Kozak, DO

Dr Kosak is a poor choice for a doctor. His primary focus is hitting some number in competition with other doctors. My evaluation was inconclusive and now I see he was just guessing about my condition and treatment. I agreed to a spinal fusion L-3 thru S-1. Post surgery was an absolute joke as he hit his number and got paid so there was no to give me any attention or address my pain. I was desperate and needed relief, so I took his advice. Bad mistake. I can prove it by providing the paper where he showed ~97.5% success rate and most likely uses the same numbers today. Why didn't my failed back surgery lower his number? Because he ignored me and rejected my pain and concerns. I am sure he didn't consider my case as a failed back surgery. Even though I am very much worse off relying on this jerk. Stay away.

I felt like was just a number. had to call multiple times to get them to return my calls. when I got my appointment Dr. Kozak came in and said maybe 10 words to me. said he needed and MRI which I understand but I can not schedule it myself. So then I waited another 3 days for the phone call back just to schedule the MRI. 3 days after MRI and 3 phone calls to Billy I finally got a call back with results (degenerative disc tear and herniation) said we would do injections and if that don't work then would talk to Dr. Kozak again. After making multiple calls finally get a call back but still no appointment for the injection. After 2 weeks of zero help finally had to change Dr. I don't know what is going on at Fondren but my experience has not been what I have read or heard...........

I had suffered increasingly severe headaches and neck stiffness from age 35 to 39. I had received treatment from my family physician, a chiropractor, a massage therapist, and a neurologist. Someone recommended Dr. Kozak to me. After examining a round of imaging, he drew for me the basic problem on the paper covering the exam table. Half of C-6 was missing, and my body was trying to compensate for the spinal column collapse. After 8 hours of surgery, a bone graft, and a plate, I had a stable reconstruction. I remember Dr. Kodak’s little dance at the foot of the hospital bed when I could wiggle my toes. He told my family that he had no idea how I was functioning with the damage he found. I began the work of rehabilitation. That was 24 years ago. Dr. Kodak gave me my life back. At the time, my sons were 7 and 5. I was able to be their mother, and live to see them happily married and successful in their careers. I’m grateful for what Dr. Kozak had the courage to custom design… for me. He is forever a hero to me.

Our experiences at Fondren Orthopedics, from healthcare workers to Physician's Assistant, Billy to Dr. Kozak is A++. Wait time, minimal. Dr. Kozak has rendered me a pain free life, having had several surgeries and injections. Billy always returns calls and makes me feel important. Dr. Kozak is highly intelligent and relays issues and diagnosis in an unpretentious manner and his work is of the highest quality. I trust him completely.

He is a very professional and his concern about you health and pains are very important . He is a outstanding surgeon . I sent family and friends and they all feel the same . We all recommend Dr.Kozak

Best doctor and office staff I have ever experienced.

Dr Kozak is amazing!! I was scared to death to have the spinal fusion, but after talking to him in lengthy detail, he really put me at peace about it. He even called me on the phone himself to talk about it. He explains everything to where you understand 100%. He takes his time with you. I could not believe how immediately after surgery all my pain & issues were gone. I recommend him to everyone I know that has back issues. His PA is top notch also.

Dr Kozak is amazing! I live out of state. When I have contacted his assistant with questions, Dr Kozak himself called me. Doctors never call patients themselves. I had a spinal fusion. I was amazed at how easy this major surgery was....but it's all in your doctor.

I have been under the knife five times only twice by him. I would NOT go anywhere else. A fine doctor and a very good man
